I understand that Oxford Women’s Care Center Advocates and Coaches will make every reasonable effort to protect my privacy. However, by giving permission to be contacted, there is an increased possibility that others may become aware that I have received services or support from the Center.

By submitting this form, I acknowledge that Oxford Women’s Care Center is a nonprofit, faith-based organization. Its trained staff and volunteers provide education about pregnancy options, pregnancy decision coaching, and practical, emotional, social, and spiritual support. These services are not a substitute for professional counseling, therapy, legal advice, medical advice, or other services provided by licensed professionals.

If I am pregnant or have other health concerns, I understand that the Center recommends consulting a qualified medical professional.
